Collaborative health care practice connotes coordination of patient care across various specializations and health care settings. The fundamental objective of collaboration and coordination of care is to realize high-quality care by a multidisciplinary approach, which is essential in increasing the value and efficiency of care with reduced costs and positive patient outcomes (Reeves, Xyrichis & Zwarenstein, 2018). However, there are barriers that impede the success of collaboration and coordination in health care. Poor coordination is among the major issues in collaborative care, particularly where the providers practice outsides the geographic areas. This is where telemedicine proves to be essential. Telemedicine enables the health care providers to coordinate and work collaboratively regardless of the geographical location. It leverages digital communication technologies, facilitates communication and expresses association between health care providers and patients. Telemedicine is also crucial in enhancing collaboration and coordination of care enhancing internal communication amongst providers and provider-to-patient communication, reduce service delivery gap and addresses shortages in specialty, and eliminate fragmentation of care (Sabharwal, 2016).
In the other hand, the disadvantages of telehealth include challenges of keeping up with highly shifting and fast growing health care laws, privacy protection rules, and reimbursement policies. Moreover, the patients can only benefit from telehealth when they have appropriate technology such as computer that is capable of video conferencing and a steady source of internet. However, some patients may are incapable of use the technology and may require assistance. Moreover, there are possibilities of technical difficulties such interruptions in sound and video, which may disrupt telehealth visit (Ellimoottil et al., 2018). On the other side, the advanced registered nurses play crucial role of responding to the needs of diverse and underserved populations. Nurses can also conduct provide patient consultation. They can also engage virtually with physician while providing health care patients in community. Overall, nurses help in coordinating virtual care between health care providers and patients.
