Historical or the baseline budgeting process builds on the organization’s historical information and incorporates the available previous performance of the operations (DeNisco, 2021). This is the best budgeting approach since it is not time consuming and is less likely to result in omission of major expenses on the budget. In addition to these advantages of the baseline budgeting approach, the disadvantages of the zero-based budgeting also a major reason why the baseline approach is the most used in the health care sector. For example, the ZBB approach is characterized as time consuming and requires routine justification of the expenses (DeNisco, 2021).

Baseline budgets are utilized to project direct and indirect costs for a project, and this budget relies on historical data to project the budgeting process in a predictable operation (DeNisco, 2021). At the same time, zero-based budgeting requires looking at each expenditure and providing a rationale or justification. The explanation that can apply to budget decisions is an analysis of cost, benefit, alternatives, data, and measures of performance (DeNisco, 2021). Both methods have their pros and cons. However, this project focuses on a new healthcare facility that lacks established workflows and financial history. A baseline budget relies on an established process and specific projections to create appropriate funding for the new facility. ZBB would provide the most valuable because it forces the group to justify expenditures and assess factors that can indirectly or directly affect the budget. Because it is a budget for a new facility, no historical information can be used to create a baseline budget, making this method inappropriate for this project.

To implement the innovation, a nurse leader must have strong interdisciplinary cooperation and complex change management skills. All registered nurses (RNs) are transformational leaders who fostered and encouraged in the workplace by using innovations such as technologies, techniques, and policies to offer greater, more affordable healthcare to patients and the general public. To build an atmosphere that promotes and encourages innovation, nursing managers and leaders are necessary (Aldridge, 2021). To optimize outcomes, nursing leaders encourage all participants in interpersonal communication to effectively incorporate and create change. To guarantee staff engagement and actively involve others in innovative technological planning, the nurses should ideally promote an environment of shared decision-making. Clinical nurses can implement the innovative care model by closing gaps, solving problems, and advancing ideas in the direction of new obligations and rules. They may develop a culture of creativity among their employees, communicate expectations regarding job commitments and acceptable risk levels, and be prepared to accept failure.
To advance the nursing process, educators and clinicians must work together more effectively. The partnership between advanced nurse practitioners and other partners in the healthcare business can be positively impacted by innovative care models in a variety of ways. For instance, they can support better collaboration and communication between nurses and other healthcare professionals. They can also encourage nurses and other partners to share their best business practices (Weintraub & McKee, 2019). Finally, innovative care models can contribute to raising the standard of patient care by encouraging cooperation between nurses and other stakeholders. Regular meetings can promote collaboration between advanced registered nurses and other stakeholders regarding any breakthrough in healthcare. In these gatherings, nurse leaders and other interested parties exchange ideas on how to work effectively together to improve patient care.

A patient care delivery model is defined as system of organizing delivery of care to meet the needs of the patient which includes work delegation, resource utilization, communication, clinical decision making, and management of the system (Weberg et al., 2019). An effective care delivery model should deliver quality patient care and outcomes by having competent health caregivers meet the needs of the patients with documentation reflecting the quality of care provided. Some guiding principles on effective health care delivery models include having a system approach by all disciplines collaborating care towards a common goal, continuity of care services through effective care coordination, promotion of shared interdisciplinary knowledge, and promotion of patient safety, experience and quality outcomes (Weberg et al., 2019).
There has been numerous care delivery models used like primary nursing, functional nursing, individual nursing and team nursing. These care models are effective in varying degrees, however, as the care environment, shift in roles and scope of practice, added specialization and interdisciplinary approach changes, it has been apparent that more consideration has to be incorporated into the care delivery models to be effective. It has to be one that is driven by principles, coordination of efforts, and achievement of value-based outcomes (Weberg et al., 2019)
To address the rapidly changing healthcare system, there is increased emphasis on care delivery models that are flexible, cost-effective, and which integrates services from hospitals with primary health care and social services as well as the integration of digital technology and telehealth services (Rapport et al., 2022).
So what is the best care delivery model? In patient centered organization with high focus on quality of care, more than choosing a model, it is important to ensure that satisfaction of the patient care needs are met. This may be achieved through innovative care models that are combination of the established traditional care models. Thus it is part of the nurse leaders role to recognize the strengths of their nursing team including the other interdisciplinary care team members, and adopt the work method that is based on the way care is planned, and the way the nurses deliver and evaluate care (Pairreira et al., 2021)..

Innovation and creative thinking are necessary to guide and shape the future of the healthcare system to better meet the needs of its consumers. In order to support innovation, clinical leaders must be open to new ideas, have courage to challenge the status quo, be comfortable with risk and have a clear understanding of their own strengths and weaknesses (Weberg, 2019). One example of an innovative care model that has grown tremendously during the pandemic is the use of telehealth and virtual visits with healthcare providers. Telehealth’s benefits include the ability to connect with more patients for shorter and more flexible visits, eliminate the difficulties associated with traveling to appointments, less missed work or school time, easier access to specialists and mental health care, to name a few (Prickett, et al., 2022) Using this modality also runs the risk of missing physical signs and symptoms, and technology access challenges.
Nurse leaders and advanced registered nurses can be influential in implementing and supporting innovative care models. For example, advanced registered nurses in the clinical nurse specialist (CNS) role can research and promote evidence-based practice modalities of providing care that improve upon current systems. By leveraging their expertise, the CNS can convince upper management that evidence supports the new model and gain their support for the change. In order to successfully implement innovative models of care, nurse leaders must collaborate with other stakeholders to gain their support in relation to adopting policy changes, absorbing costs and providing professional support for achieving successful implementation.

Innovation is about being resourceful, improve the cost and quality of resources by either creating new ones or enhancing what is available. Nurse leaders who implement innovative care models have the role of researching available models and identifying which would best suit their current climate. They will look at culture, work practices, learning modalities, current technology and staffing concerns. According to Noles et al. (2019) innovative models should be including in the curriculum for nursing leaders. Within the study from Noles et al. (2019) it states that nursing leaders had less innovative ideas than that of Certified Nursing Leaders and suggested that nursing leaders should place CNL’s and allow innovation from nursing at all levels. Specifically, with CNL’s they are trained and can provide more depth and are suggested that if used properly can help redesign healthcare systems that will improve health, lower cost and so much more.
The utilization of innovative models of care can foster teamwork interprofessional. Through the eyes of staff that is utilizing these models they can share their experiences and have solutions that fit their climate and patient need. Kelly (2019) points out that through nurse collaboration and innovation the CNP program and pain scale came to fruition. Because of nurses can forge new pathways these changes have also changed how care is provided.
