**Discuss the questions that would be important to include when interviewing a patient with this issue.
Why are you seeking help today? I’ve had this pain in my lower abdomen for 5 days, pretty mild pain (points to the suprapubic region). Then yesterday it seemed to move to my left lower side, in the pelvis and it has been getting worse and worse.

Do you have pain anywhere? If so where? Yeah, really bad pain here… (points to LLQ/pelvis).
Have you been having any fevers? No. Funny, I haven’t had a fever, but I have had chills and sweats.

When did your vaginitis/vaginal discharge start? It started around the time my abdomen started to hurt.
Have you ever had discharge like this before? No. I don’t think so. Well, maybe it is sort of like the stuff that came out when I had the chlamydia a few years ago. This seems worse or different somehow.
Does anything make your pain better or worse? Um, standing on my job as a cashier for a long time has been a killer — totally worse. My job as a cashier, you know, on my feet all day, really makes it so bad I had to leave work. Yesterday, I took the whole day off and laid on the couch. I thought I could manage work today, but I had to leave after just an hour. The pain was so bad. Now just about any movement and walking around is really bad. Nothing makes it better.

**Describe the clinical findings that may be present in a patient with this issue.
The clinical findings that may be present in a patient with TOA includes the sudden onset of lower abdominal pain, chills, dyspareunia, fever and vaginal discharge (Carter & Garmel, 2016). Other symptoms may include nausea, vomiting and abnormal vaginal bleeding. Women with a ruptured TOA can present with signs and symptoms of an acute abdomen and sepsis.
Findings related to Tubovarian Abscess
**Are there any diagnostic studies that should be ordered on this patient? Why?
The initial evaluation includes routine blood work, urinalysis, vaginal and blood cultures and imaging study, with either a transvaginal ultrasound or a computed tomography scan of the abdomen and pelvis. Treatment for tubo-ovarian abscesses have at least 24 hours of inpatient parenteral treatment with antibiotics, and that they may require surgery. If surgery becomes necessary, pre-operative administration of broad-spectrum antibiotics is started and removal of the abscess (Carter & Garmel, 2016). After discharge from the hospital, oral antibiotics are continued for the length of time prescribed by the provider to assure the infection is cleared.

**List the primary diagnosis and three differential diagnoses for this patient. Explain your reasoning for each.
Primary diagnosis: Tubo-ovarian abscess (TOA) is an inflammatory mass found in the fallopian tube, ovary and adjacent pelvic organs and is one of the late complications of pelvic inflammatory disease. It consists of an encapsulated or confined ‘pocket of pus’ with defined boundaries that forms during an infection (Silva et al., 2015). A Tubo-ovarian abscess is a life-threatening condition that can present with sepsis and shock if rupture occurs. This is the most likely diagnosis based on Ms. Phillips’ clinical presentation, risk factors, and diagnostic testing results.

Hello class and welcome to the class and I will be your instructor for this course. This is a -week course and requires a lot of time commitment, organization, and a high level of dedication. Please use the class syllabus to guide you through all the assignments required for the course. I have also attached the classroom policies to this announcement to know your expectations for this course. Please review this document carefully and ask me any questions if you do. I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. I advise you look at the requirements for these assignments at the beginning of the course and plan accordingly. I have posted the YouTube link that explains all the class assignments in detail. It is required that you watch this 32-minute video as the assignments from week 3 through 5 require that you follow the instructions to the letter to succeed. Failure to complete these assignments according to instructions might lead to a zero. Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.
I understand this is a lot of information to cover in 5 weeks, however, the Bible says in Philippians 4:13 that we can do all things through Christ that strengthens us. Even in times like this, we are encouraged by God’s word that we have that ability in us to succeed with His strength.
